Note: These forms will be prepared in full by the student or the advisor and will be sent to the Social and Human Sciences Research Ethics Committee by the student's advisor with a cover letter.
*Applications outside the institution (excluding KBU personnel and students) can be made by hand to the Social and Human Sciences Research Ethics Committee together with the Ethics Committee Application Form.
Application for Permission to Conduct Surveys from Non-Institutional Places/Institutions/Organizations

Documents to be attached to the Permission Request Form in the Surveys of the Institutions Affiliated to the Ministry of National Education;
Students who will request a survey permission should apply by preparing the forms which they will fill out at https://ayse.meb.gov.tr/basvurudev/. They also need to attach Ethics Committee Decision and Survey Studies to the annex of the permission request forms.
Documents to be Added to the Permission Request Form in the Surveys to be Made in Public Hospitals;
Important Note: The Provincial Health Directorate does not give the Survey Work Permit without the documents given below, and our students must fill out these forms completely.
* Hospital Research Preliminary Permission Document
* Scientific Research Application Form
*Consultants whose survey application has been completed the Social and Human Sciences Research Ethics Committee shall fill the Permission Request Form with its attachments and apply to our Directorate in writing.
*Those Request forms which are not complete or do not specify the institutions to be surveyed are not specified in detail will not not be processed by the General Secretariat.
*After the necessary permissions are completed, the student can start the Survey work.
